Annually, a physical examination must be on file with Health Services prior to the first day of the first season and expires at the end of each school year.  For your convenience, we will offer a school-wide physical during the summer.  Staffed with physicians, physician’s assistants, physical therapist, nurse, and student athletic trainers, we offer a thorough examination that fulfills the requirement for school physicals and/or sports physicals.

Physicals

The physical needs to be completed by an MD or DO and dated after June 1^st.

CIF ethics form is required prior to the first athletic event and expires at the end of each school year.

Please submit the physical examination and CIF ethics form to Health Services addressed to Mr. David (Richi) Klapmeier.
