REMINDER: ALL SPRING SPORT ATHLETES: Please make sure that you have completed ALL required forms to play sports! This includes: Annual Physical using SAINTS PHYSICAL FORM, you need a physical EVERY year -your physical from last year does not count. Front page completed and signed by you and parent. Back page completed and signed, with a stamp from your doctor. If you are unsure whether you have completed all your required forms, Please check in with your head coach to determine whether you are cleared or not.
