All incoming students (freshmen and transfers) are required to have a current, valid physical prior to the start of the school year. After the first year of attendance, all students trying out or participating in a sport must have a current/updated physical prior to the first day of tryouts. 

 

The Saints Physical Form should be printed and 

• Medical History (page 1) completed and signed by parent/guardian
• Back page completed and signed by physician 
  • Make sure the form is signed AND stamped by the physician
• Once complete, you may submit the physical form to the school by:
  • Scanning and emailing to Zorinna Aceron, Athletic Assistant at zaceron@sahs.org
  • Faxing to the school (619) 282- 1203
  • Mailing or dropping off a hard copy to the front office (always keep a copy for your records)
